At Project Health, our Steady @ Home Program helps older adults prevent falls, build strength, and stay independent at home. Services include home safety assessments, PT-led balance and strength training, personalized mobility practice, and family education. Medicare may cover eligible participants, ensuring safe, effective, and supportive care in the comfort of home.
Who Qualifies (Medicare Requirements)
Services are available to Medicare beneficiaries who are 65+ (or otherwise eligible), under a physician’s care, require intermittent skilled nursing or therapy, and meet Medicare’s homebound criteria with a signed Plan of Care. Coverage depends on Medicare eligibility, not age alone, ensuring care is appropriate, approved, and personalized.
